Pre-Registration Deadline is APRIL 26, 2013.
Your sponsor form and all collected donations should be turned in at the Walk event.
On-site registration will be available for an extra charge.

You will add up all fees and promotional-items pre-orders on page 2.

T-SHIRT INFORMATION:

• Each REGISTRATION will receive ONE complimentary event t-shirt.

• Additional t-shirts may be pre-ordered on the registration form. Complimentary and pre-order t-shirts
will be available for pick-up on the day of the walk.

• To ensure that we have adequate quantities for pre-ordered and complimentary t-shirts,
registrations MUST be received no later than April 26, 2013.

• Please indicate quantities of t-shirts in each size class on the registration form. PLEASE
INCLUDE the complimentary shirt in those quantity counts.

• NOTE: Youth S-XL and Adult XS-XL shirts are $8.00 each; Adult XXL, XXXL shirts are $10.00.

This event is a fundraiser on behalf of PWSA of Ohio and PWSA (USA). Asking
your family, friends, and co-workers to donate in honor of your participation not
only raises awareness of Prader-Willi Syndrome, it also helps raise money! PWSA
of Ohio needs funding to help the Ohio PWS community and the PWS population
nationwide. Please use the “Sponsor Page” on page 3; collect all sponsor money
